Instructions:
1.) Complete all information requested on this form and submit.
2.) A return authorization number will be emailed to you at the email address you provided.

Important - Please Note
- Returns must be made within 30 days of delivery in "like new" condition.
- Returned items must be in original factory packaging.
- Any earpiece that fits inside the ear is not returnable if it has been removed from the sealed packaging.
- A 20% restocking fee will be charged on any product returned unless merchandise is exchanged for an item of equal or greater value.
- Shipping charges will not be refunded.
